On this website (www.straightouttapvc.com) you may have seen a section called “The Hub.” This is our team member section that has some fun information, events, and our team directory. As one of our newest team members, we are excited to add you to the Team Directory! To do so, we will need a little bit of information from you:

Please send an EMAIL to rmiller@ohiovisioncare.com with the following info:

• The best phone number team members can reach you at

• A fun fact about yourself

• Your birthday

• And a photo of yourself that you are comfortable having displayed in the team directory. Since we have 4 office locations and lots of fantastic team members, having a face to match the name is super helpful! Please make sure the photo is in jpeg or png format.

Once you send that email, please allow a week or so for Raquel to add that info to the Hub Team Directory. :) That's it for this task!
